The Children’s Behavioral Health Collaborative (CBHC) is a collective impact initiative to improve behavioral health outcomes for children and their families throughout New Hampshire. We have served as technical assistant and evaluation workgroup leader for NH’s CBHC since December 2015. In that role, we have assessed the CBHC network and New Future’s functioning as the collaborative’s “backbone organization” for this Collective Impact initiative. We have developed structures, processes, and tools to be used by the CBHC’s workgroups, and the evaluation workgroup also identified shared impact indicators in 2016. Finally, in partnership with evaluation workgroup members, we developed a shared measurement plan/platform for the CBHC to monitor workgroup, project and indicator progress, to be implemented in 2017.
CBHC Logic model
This model shows the overall theory of change that frames work planning for the CBHC, based on the collaborative’s 2013 strategic plan.
