Report It
Sexual Misconduct and Pregnancy-Related Conditions (Title IX)
This form is for reporting incidents of sex or gender-based discrimination, sexual misconduct, and to request Academic Adjustments (accommodations) for pregnancy-related conditions. Reports can be submitted by any member of the Antioch community.
Disruptive Behavior Report (Student Conduct)
Use this form to report student conduct that interferes with classroom instruction or another student’s ability to learn. Reports may be submitted by students or employees.
Bias Incident Report
Use this form to report incidents of discrimination, harassment, or bias-motivated behavior involving Antioch students, staff, or faculty.
Early Alert Referral
Report concerns about a student’s well-being, behavior, or academic performance. This form allows faculty, staff, or students to refer someone for outreach and support.

Use this form to request academic or campus accommodations for a documented disability.
Student Services Request Form
Students: Use this form to connect with Student Services for help with admissions, financial aid, registration, student records, billing, academic support, and more.
Faculty/Staff: Use this form to refer a student who could benefit from academic or university support services.
Veteran and Military-Connected Student Request
Veteran and military-connected students can use this form to request academic, financial, or other university support services.
Emergency Student Support Fund Request
This form is for students who need short-term financial assistance due to unexpected hardships (e.g., housing, food, transportation, academic tools).

Reports are kept as private as possible and shared only with those who need to know in order to respond appropriately.
