Healthy Monadnock 2020 (HM2020) is a collective impact inspired healthiest community initiative in the Monadnock region of New Hampshire. Cheshire Medical Center/Dartmouth Hitchcock Keene (CMC/DHK) founded the HM2020 initiative and serves as its “backbone organization.” Through a data-informed, community-driven process, HM2020 developed four strategic pathways. These pathways consist of evidence-informed strategies that are brought to life by community partners, through implementation of complementary sets of interventions (i.e., programs, projects, and policies), to create lasting environmental change. These strategic pathways are directed at four high leverage targets – health access and quality, health behaviors, social determinants of health, and social capital – all of which are ultimately directed toward achieving the intended impact, of becoming the healthiest community in the nation by 2020. BHI served as external evaluators for HM2020 from May 2011 to December 2015.
